Please complete a registration for each student you wish to enroll for after-school activities. Billing for after-school clubs will be processed through Alma, our parent portal. There is a minimum of 6 students required for us to offer the club and class size limits. Health Form – Fill out a 2-page health information section for each child on this form. It may be helpful to gather health insurance cards, doctor and dentist contact information, and emergency contact numbers before you begin. If your child requires an Individual Health Care Plan (due to chronic medical conditions) or a Food Allergy Action Plan (FAAP), please read this important message from Nurse Chouljian. Updated Records – Each student must have an updated immunization form (new and K–5 students only), updated physical form (new students and rising 6th grade students only), and a copy of their birth certificate (new students) on record before the first day of school. By Florida law, students are not allowed to enter school until these forms have been submitted. Documents can be submitted here. Technology Form – Complete one entry per student. Lunch Options Bagel King’s Healthy Kids’ School Lunch offers up to 8 different lunch options daily, including gluten-free, dairy-free, and meat-free options. They also buy locally grown fruit and vegetables. All bread, bagels, and desserts are made in-house daily! Potbelly Sandwich Works is available 5 days a week! Students and teachers are able to get a lunch box that includes a sandwich, chips, cookie and apple sauce. Asia Kitchen offers Asian-inspired cuisine for our 4th grade students through middle school and staff during the 2nd lunch period only. State of Florida Educator Misconduct Policy All employees and administrators are obliged to report misconduct by instructional personnel and school administrators, which affects a student’s health, safety, or welfare. Examples of misconduct include obscene language, drug and alcohol use, disparaging comments, prejudice or bigotry, sexual innuendo, cheating or testing violations, physical aggression, and accepting or offering favors. Employees’ misconduct reports should be made to the appropriate administrator along with the Human Resources department reports of misconduct committed by administrators to Melanie Taplin, Regional Director of Spring Education Group. Legally sufficient allegations of misconduct by Florida-certified educators will be reported to the Office of Professional Practices Services. The school Employee Handbook posted policies and procedures for reporting misconduct by instructional personnel or school administrators that affect a student’s health, safety, or welfare.
